You can complete your diaper request online by completing the registration form below or when you come into the office on the days listed below. 

 

Beneath are two links to our registration forms that provides us with information we need about you and your baby. Please answer all questions truthfully and completely. This form provides us with vital information to apply for grants so we can continue to provide diapers. Rest assured that your information is protected, and we will never sell your information to anyone.

 

Please note that you can only request clothing once every six months.

Next visit our offices located at 117 Park Avenue, Hamilton, OH 45013, on Wednesdays from 4:00-6:30p or Fridays between 11:00a -1:30pm.
